Clinical Outreach Specialist

JOB SUMMARY:

The Clinical Outreach Specialist will serve as a key liaison between our organization and the broader healthcare community of Nassau County, FL. The Clinical Outreach Specialist is responsible for building, maintaining, and nurturing relationships with clinical professionals, treatment centers, hospitals, and referral sources to increase awareness of our services and expand our referral network.

DUTIES AND RESPONSIBILITIES:

  • Outreach & Relationship Building
    • Establish and maintain strong relationships with clinicians, hospitals, treatment providers, discharge planners, care navigators and referral partners.
    • Identify and pursue new outreach opportunities to grow the referral network.
    • Representing NCCOA with the highest degree of integrity and professionalism.
  • Education & Promotion
    • Communicate the mission, values, and clinical offerings of the organization to external stakeholders.  Specifically promoting our home healthcare agency “Advocate Home Healthcare,” our Adult Day Health Center (ADHC) “The Club,” and Caregiver services.
    • Assist with and/or deliver small group presentations, attend networking events, conduct educational workshops, coordinate / facilitate speaker panels, and represent the organization at conferences and community events.
  • Collaboration & Support
    • Work closely with Advocate Home Healthcare, The Club Adult Day Health Center, marketing, and Caregiver Support teams to align outreach strategies with program capabilities.
    • Collaborate with Advocate Home Healthcare, The Club Adult Day Health Center and Caregiver Support managers/ staff to grow enrollment in each respective program and achieve enrollment goals.
    • Provide feedback from the field to internal teams to help improve services and outreach effectiveness.
    • Support the Adult Day Health Center /Caregiver Support services programs as needed. Collaborate with the ADHC Manager in order to achieve desired outcomes.
    • Respond to inquiries from the public, providing accurate program information and referrals to appropriate services.
    • Other duties as assigned.
  • Reporting & Documentation
    • Track and report on outreach activities, referral trends, and achieved outcomes.
    • Maintain an outreach tracking system to manage contacts, referrals and follow-ups.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Strong understanding and knowledge of:
    • Home healthcare, associated systems and payer sources.
    • Community services and resources available to older adults.
    • Home & community based services.
    • Alzheimer’s disease and other forms of dementia.
    • Caregiver needs and resources.
  • Excellent interpersonal, communication, and public speaking skills.
  • Ability to travel locally or regionally as needed.
  • Ability to multi-task.
  • Self-motivated, goal-oriented, and comfortable working independently.
  • Proficient in Microsoft Office.
  • CPR and First Aid Certified.

SUPERVISION:

  • Performs duties under the supervision of the Chief Administrative Officer.

WORK EXPERIENCE / REQUIREMENTS:

  • 2+ years of experience in healthcare, healthcare outreach, clinical marketing, public administration, business development, social sciences or related role.
  • Clinical background (e.g., LPN, LCSW, LMHC, RN) a plus but not required.
  • Existing relationships in the aging services, behavioral health or healthcare community.
  • Experience working in healthcare, home & community based services, sales/marketing, holistic wellness programs and with older adults.
  • Reliable transportation and valid driver’s license.
  • Must be able to pass the Level 2 background check required by DOEA.
  • Must adhere to Drug Free Workplace standards.

EDUCATION: 

  • Bachelor’s degree in Nursing, Healthcare Administration, Public Health, Social Sciences, Gerontology or related field.

 

PHYSICAL WORK REQUIREMENTS: 

  • Able to sit and work at a computer keyboard for extended periods of time.
  • Able to stoop, kneel, bend at the waist and reach on a daily basis.
  • Able to perform general office administrative activities: copying, filing, delivering and using telephone.
  • Able to life 30 pounds.
