Resource Coordinator

Summary/Objectives:

The Resource Coordinator is responsible for Barnabas participant’s needs’ assessment and coordination of appropriate services to assist them with addressing their health, financial, social, emotional, and educational or other needs that will facilitate their achievement of overall good health and self-sufficiency.


Essential Functions:

1. Screens and interviews participants and assesses overall needs, barriers, and assets.

2. Provides information to individuals and families on services and works collaboratively with them to develop a plan to address needs and move toward a level of improved health and self-sufficiency.

3. Works with individual participants to develop career and vocational goals, plans & strategies to build a path for improved financial stability.

4. Utilizes evidence- based curriculums with participants on 1:1 basis for life skills building and financial literacy building. Facilitates resume writing & employment interviewing instructions.

5. Meets and/or contacts participants routinely to evaluate progress in meeting the goals and objectives of their plan. Coaches participants, advocates on their behalf and otherwise encourage and support their efforts towards achieving their goals.

6. Coordinates services and makes appropriate referrals to address the needs of participants, including, but not limited to, mental health services, financial assistance/counseling, childcare, educational services, employment, vocational or job training, health care, transportation, affordable housing, etc.

7. Ensure a minimum of one monthly contact with all active participants.

8. Develops relationships with property owners and service organizations in order to benefit participants of Barnabas program(s); negotiates rental costs(s) and terms of housing and other negotiations on behalf of participants.

9. Maintains routine administrative responsibilities including but not limited to referring participants to Barnabas programs, enrolling participants in Hope Builders Program and completing relevant program assessments, service plans and documenting all participant contact in Barnabas’s client database.

10. Completes all documentation for processing to ensure timely payments to landlords/vendors and other service providers.

11. Assists participants in applications for benefit assistance such as Medicaid, Supplemental Nutrition Assistance Program (SNAP), SSI/SSDI, etc.

12. Works collaboratively with an interdisciplinary team of Barnabas personnel to address challenges with participants who require additional support, such as primary medical care, dental care, food assistance, behavioral healthcare, etc.

13. Maintains flexible hours to meet participants needs.

14. Completes monthly reports as assigned.

15. Performs other duties as assigned.

Supervisory Responsibilities:

This position has no supervisory responsibilities.


Work Environment:

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.


Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.


Position Type and Expected Hours of Work:

This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5:00 p.m.  Occasional evening and weekend work may be required as job duties demand.


Travel:

No travel is expected for this position.


Required Education and Experience:

High School diploma or GED.  Experience working with low-income and/or homeless families.    


Preferred Education and Experience:

Bachelor’s degree in social work, counseling, psychology, or related field.
